Costs start as low as $90.  A DOT physical costs $150 for an individual. If there are ten or more drivers from the same company getting examined at roughly the same time, the cost is reduced to $90 each. There is also a mileage fee of $3 for every mile. The first 15 miles from New Providence, NJ are complimentary. Payments can be made via cash, company check, or credit card.
